Ø  The fill legal name of organization is always requirements. If there is any other name that should registered federally as well.

Ø  The incorporation number should be given. If it is not an incorporation. In case of partnership and co operative if there are no identification number then you need to give declaration details of co-operative documents as well

For CTLS, you must submit controlling organization program noted as parent corporations. The account ID of each application must be submitted to health Canada.

Ø  If your firm is a partnership firm then you need to submit agreement of partnerships also. In case of partnership or co-operatives, partnership or cooperative documents are required. 

Ø  As a part of submission chart. An organizational chart is required for your company. This will Must demonstrate the relationships between senior positions within the organization and the various controlling individuals or entities, if applicable. This must include all names and titles of senior management positions such as directors and officers of the organization and any controlling individual or entity, if applicable. Does not need to include the site-specific organizational information (e.g., the site head of security, master grower, QAP). This specific organizational information is required as part of a specific application and is to be included in the OSP.
